We currently have immediate openings for the following full-time positions:

Patient Care Coordinator responsible for assigned team intake coordination and the planning, implementation, delivery and supervision of direct clinical services to patients and families.  Requirements:  RN licensed in Virginia; 5+ years relevant nursing experience; 2+ years in supervisory role; demonstrated ability to manage all aspects of terminal care, staff supervision, documentation, service coordination and case management.

RN Case Manager responsible for the care of terminally ill patients through professional management of cases assigned and the coordination of services offered by health team members.  Applicants must be an RN licensed in the state of Virginia, have 2+ years relevant nursing experience, demonstrated ability to manage all aspects of terminal care, health team service coordination, documentation and case management.

RN (evenings 3-11) at Hospice House.  This 100-year-old house has been remodeled and is now the home for a total of eight Hospice residents.

Supervisor, Social Workers with a clinical concentration and an interest in working with terminally ill patients and their families.  Must have exceptional communication and organizational skills to work collaboratively as a member of the interdisciplinary team of physicians, nurses, spiritual caregivers, volunteers and aides.  Two plus years of supervisory experience within the area of professional practice is required.  Will provide psychosocial assessments, direct casework and counseling services to patients and families in Charlottesville and surrounding counties.

Coordinator of Volunteer Services   This position is responsible for supervision and coordination of volunteers in all aspects of services within Hospice of the Piedmont programs in Charlottesville and surrounding counties.  Also will oversee program development and implementation.  Successful candidate will possess a Bachelor's degree in education, social work or other humanities; excellent administrative abilities; outstanding interpersonal skills, and direct experience in recruitment, training, and supervision of volunteers.

Grief and Loss Coordinator/Counselor  Provides bereavement services for Hospice families and bereaved community persons within our service area.  The position requires a Master's degree in social work, counseling, or related field.  Clinical experience with individuals, groups and families and experience in bereavement counseling is also required.  Successful candidate will have strong interpersonal skills, communication skills, and computer skills.

Certified Nurse Aide (CNA) to serve patients in Charlottesville and surrounding counties.  As a member of the health care team, candidate will provide personal care and attendant services ensuring comfortable, safe, total patient care.
