Positions in Charlottesville and the surrounding area:

Admissions Manager (Full time) – The Admissions Manager serves as a member of the Admissions/Intake Team. S(he) is responsible for the oversight of the Admissions/Intake Department; duties to include coordinating referrals; assisting families and referral sources regarding inquiries about hospice philosophy and services, along with creating and maintaining relationships with area providers. Must be licensed as a Registered Nurse in the Commonwealth of Virginia. Five years of nursing experience including two years at a supervisory level preferred. Hospice experience required.

CNA (PRN – Field) – Must be registered by the Virginia Department of Health Professions as a Certified Nursing Assistant. Under the direction of a Registered Nurse, provides personal care services to patients in home and/or facilities that reflects the Hospice philosophy of care. One year’s relevant experience required. Basic proficiency in word processing software helpful.

CNA – Hospice House (PRN) – Must be registered by the Virginia Department of Health Professions as a Certified Nursing Assistant. Under the direction of a nurse, provides personal care services to Hospice House patients. Assists with meal preparation and housekeeping as needed. Hospice experience preferred. Knowledge of skills relevant to grief and loss helpful. Basic proficiency in word processing software helpful.

Clinical Services Manager (Full time) – The Clinical Services Manager supervises and directs the care given by a team of RN case managers, social workers, chaplains, home health aides, and volunteers to hospice patients and their families. Must be a Registered Nurse in the Commonwealth of Virginia with five or more years of relevant hospice clinical experience preferred. Must have knowledge of EMR systems and be able to utilize the system in the daily management and oversight of the interdisciplinary team’s performance.

Director of Finance (Full time) – Manages the general accounting function, financial planning, audit and compliance; prepares annual budget; manages relationships with financial institutions and investment managers, and contract services. BS/BA Degree and CPA required. Seven or more years relevant experience, preferably in non-profit and healthcare, with strong knowledge of GAAP. Proficiency in MS Office software required. Medicare/Medicaid billing experience and familiarity with Electronic Medical Records a plus.

Director of Technology Services (Full time) – The Director of Technology Services provides strategic evaluations of information system applications, computer and network hardware, and data communications equipment. Recommends changes as necessary to ensure the appropriate technology is aligned with the goals and objectives of the department and HOP. The incumbent will manage all aspects of technology including technical design, systems development, construction, and delivery of enterprise-wide technology solutions. He/she will ensure continuity of computer services for computer users throughout the organization through planning, technical leadership, and project coordination. Will be an important part of the Executive Leadership Team and will report to the CEO. Bachelor Degree in computer science, computer information systems or equivalent combination of education and experience required. Five or more years of experience in an information technology-related position required. Prior experience in a supervisory capacity preferred. Experience managing high-level network administration environment and connectivity of Microsoft Windows Server, Backup Exec, email servers including MSExchange Server, and Google Gmail, Antivirus (server and client), and WindowsXP preferred.

Nurse Practitioner (Part Time) – Provides direct care and clinical expertise to individuals, families and groups in the home or facility. In collaboration with the Medical Director, Associate Medical Director(s), and other staff, she/he is responsible for the assessment, planning, implementation and evaluation of current hospice patients. Must be licensed to practice in the Commonwealth of Virginia; certified as a Clinical Nurse Specialist or Nurse Practitioner; certified by a national body (ANCC, NBCHPN, AANP), palliative care ARNP preferred. Minimum of five years nursing experience, clinical nursing care in geriatrics, long term care and/or hospice preferred.

RN Evening/Weekend Services (Full time) – Must be a graduate of an accredited nursing school and licensed as a Registered Nurse in the Commonwealth of Virginia. Minimum of two years of recent, varied work experience as a nurse, Hospice experience preferred. The Evening/Weekend Services Nurse assures the delivery of high quality patient care through timely and responsible management of patient/family needs by responding to telephone calls and making visits as needed to assess patient needs and provide nursing care and emotional support.

Positions in Culpeper and surrounding area:

Registered Nurse (PRN) – Graduate of an accredited nursing school and licensed as a Registered Nurse in the Commonwealth of Virginia. Minimum of two years of recent, varied work experience as a nurse, Hospice experience preferred.
Responsible for patient care through professional management of cases assigned and coordination of services offered by all health team members to assure quality care consistent with Hospice policies/procedures and the effective use of resources.

RN Case Manager (Full time)– Responsible for patient care through professional management of cases assigned and coordination of services offered by all health team members to assure quality care consistent with Hospice policies and procedures. Must be a licensed RN in the Commonwealth of Virginia with two years of relevant nursing experience. Demonstrated ability to manage all aspects of terminal care, health team service coordination, documentation, and case management is necessary. Must be comfortable working as part of an interdisciplinary team.
