Positions in Charlottesville and the surrounding area: 

Hospice Aide (Full time, 7 p.m.-7 a.m., Hospice House) – Must be registered by the Virginia Department of Health Professions as a Certified Nursing Assistant. Under the direction of a Registered Nurse, provides personal care services to Hospice House patients that reflects the Hospice philosophy of care. One year’s relevant experience required. Basic proficiency in word processing software helpful.

Hospice Aide (Full Time, Field) – Must be registered by the Virginia Department of Health Professions as a Certified Nursing Assistant. Under the direction of a Registered Nurse, provides personal care services to patients in home and/or facilities that reflects the Hospice philosophy of care. One year’s relevant experience required. Basic proficiency in word processing software helpful.

LPN (Full time, On call) – Must be licensed as a Licensed Practical Nurse in the Commonwealth of Virginia. One or more years of relevant nursing experience required. Under supervision of a registered nurse, delivers quality patient care and comfort measures to patients, emphasizing physical, spiritual and emotional needs. Basic proficiency in word processing software helpful.

RN Case Manager (Full time) – Responsible for patient care through professional management of cases assigned and coordination of services offered by all health team members to assure quality care consistent with Hospice policies and procedures. Must be a licensed RN in the Commonwealth of Virginia with two years of relevant nursing experience. Demonstrated ability to manage all aspects of terminal care, health team service coordination, documentation, and case management is necessary. Must be comfortable working as part of an interdisciplinary team.

Team Coordinator (Full time) –Supports the Clinical Services Manager in their role of leading and managing a team providing direct care to patients and families. This includes triaging incoming calls, acting as primary liaison for transport and equipment needs and tracking Medicaid and Medicare re-certifications. High school graduate required; college experience and LPN preferred. Recent experience in medical records system for home care/hospice preferred. Should be able to demonstrate knowledge of medical terminology and advanced computer skills, including proficiency in word processing and excel.

Positions at the Center for Acute Hospice Care (opening fall 2015):  

Hospice Aide (FT/PT) – (Day and Night Shifts available) – Primary function is to provide personal healthcare and related services to the patient in the hospice inpatient unit setting. To assist in proving a safe and clean environment, work cooperatively with patient and family and share observations and problems with the supervisor. Works under the direction and supervision of a registered nurse.
Qualifications:

  • High School Diploma or GED
  • Current CPR Certification
  • Certification as Certified Nursing Assistant
  • Hospice experience preferred

Licensed Practical Nurse (PT) – (Day and Night shifts available) – The Licensed Practical Nurse performs direct patient care activities under the direction of an RN within the scope of LPN license and in accordance with accepted standard of practice in an inpatient unit setting.
Qualifications:

  • Licensed Practical Nurse, licensed by the State of Virginia
  • Two years (working) LPN experience, some hospice or home health experience preferred.
  • CHPLN certification preferred
  • Current CPR Certification

RN – Hospice Inpatient Unit (FT/PT) (12-hour Day and Night Shifts available) ($1000 sign on bonus for Full Time only) – Hospice of the Piedmont is looking for Registered Nurses for our brand new, 10-bed inpatient unit in Charlottesville, VA. Under the direction of the Clinical Services Manager, the RN plans, organize,s and directs inpatient hospice care. The RN identifies patient/family caregiver needs and provides nursing and supportive care in accordance with the plan of care and organizational policies and procedures.

Qualifications:

  • Graduate of an accredited nursing school
  • Licensed as a Registered Nurse (RN) in the Commonwealth of Virginia
  • Ability to manage all aspects of terminal care, health team service coordination, and documentation
  • At least 1 year of hospice, home health, med/surg or ICU/CCU experience
  • CHPN Preferred
  • Basic knowledge of Medicare/Medicaid reimbursement regulations
  • Current CPR Certification
