RN Case Manager (Full time) – Responsible for patient care through professional management of cases assigned and coordination of services offered by all health team members to assure quality care consistent with Hospice policies and procedures. Must be a licensed RN in the Commonwealth of Virginia with two years of relevant nursing experience. Demonstrated ability to manage all aspects of terminal care, health team service coordination, documentation, and case management is necessary. Must be comfortable working as part of an interdisciplinary team.
Admission Nurse (Full time) - The admission nurse will coordinate all services related to individuals that have inquired about or been referred to hospice services; provide the initial hospice presentation to patients and their circle of support, assess their understanding of the hospice benefit, actively advocate for hospice admission of medically eligible patients, admit the patient to hospice services, conduct initial nursing assessment, and attend to immediate needs of the patient and family. The nurse will advocate for and promote the gift of hospice throughout the extended care community through legendary customer service and education. Must have Associate’s or Bachelor’s degree in Nursing or equivalent from an accredited school of nursing. One year of home health or hospice experience is required. Must be licensed as an RN in the Commonwealth of Virginia, with current CPR certification.
CNA – Field (Full time) – Provides personal care services to patients and/or homemaker services supporting the caregivers. Must have high school diploma or GED. One year experience required, must be registered by the Virginia Department of Health Professions as a Certified Nurses Aide. Hospice experience is preferred.
LPN (Part time) – 12-hour shifts Saturday and/or Sunday – Would consider job share for this position, with two LPNs splitting weekend coverage. Under supervision of a registered nurse, delivers quality patient care and comfort measures to patients, emphasizing physical, spiritual and emotional needs. Cares for patients in place of RN, managing symptoms. LPNs must be licensed in Virginia. One or more years of relevant experience is required; hospice home care or nursing home experience preferred.
