Positions in Charlottesville and the surrounding area:

Admissions Nurse (Full time) – Coordinates all services related to individuals that have inquired about or been referred to hospice services. Provides the initial hospice presentation to patients and their circle of support, assesses their understanding of the hospice benefit, actively advocates for hospice admission of medically eligible patients, admits the patient to hospice services, conducts initial nursing assessment and attends to the immediate needs of the patient and family.  Requirements include a Bachelor or Associate degree in Nursing or equivalent from an accredited school of nursing, one year experience in Hospice or Home Health and must be licensed as a Registered Nurse in the Commonwealth of Virginia.

CNA (PRN – Field) – Must be registered by the Virginia Department of Health Professions as a Certified Nursing Assistant. Under the direction of a Registered Nurse, provides personal care services to patients in home and/or facilities that reflects the Hospice philosophy of care. One year’s relevant experience required. Basic proficiency in word processing software helpful.

CNA – Hospice House (PRN) – Must be registered by the Virginia Department of Health Professions as a Certified Nursing Assistant. Under the direction of a nurse, provides personal care services to Hospice House patients. Assists with meal preparation and housekeeping as needed. Hospice experience preferred. Knowledge of skills relevant to grief and loss helpful. Basic proficiency in word processing software helpful.

LPN – Field (PT 20+ – Weekends, 8 a.m. – 8 p.m.); (PRN – Weekends); (PRN); (FT) – Must be licensed as a Licensed Practical Nurse in the Commonwealth of Virginia.  One or more years of relevant nursing experience required. Under supervision of a registered nurse, delivers quality patient care and comfort measures to patients, emphasizing physical, spiritual and emotional needs. Basic proficiency in word processing software helpful.

RN (PT 20+ – Weekends, 8 a.m. – 8 p.m.); (PRN – Weekends); (PRN)- Graduate of an accredited nursing school and licensed as a Registered Nurse in the Commonwealth of Virginia. Minimum of two years of recent, varied work experience as a nurse, Hospice experience preferred. Responsible for patient care through professional management of cases assigned and coordination of services offered by all health team members to assure quality care consistent with Hospice policies/procedures and the effective use of resources.

RN Case Manager (Full time)– Responsible for patient care through professional management of cases assigned and coordination of services offered by all health team members to assure quality care consistent with Hospice policies and procedures. Must be a licensed RN in the Commonwealth of Virginia with two years of relevant nursing experience. Demonstrated ability to manage all aspects of terminal care, health team service coordination, documentation, and case management is necessary. Must be comfortable working as part of an interdisciplinary team.

RN Evening/Weekend Services (Full time) – Must be a graduate of an accredited nursing school and licensed as a Registered Nurse in the Commonwealth of Virginia. Minimum of two years of recent, varied work experience as a nurse, Hospice experience preferred. The Evening/Weekend Services Nurse assures the delivery of high quality patient care through timely and responsible management of patient/family needs by responding to telephone calls and making visits as needed to assess patient needs and provide nursing care and emotional support.

RN – Hospice House (Full Time, 7 p.m.-7a.m.) – Works as a team member in the Hospice House, providing professional nursing services to patient and family, utilizing the hospice concept.  Must be licensed as a Registered Nurse (RN) in the Commonwealth of Virginia.  Two years of relevant nursing experience required.  Nursing home/assisted living facility experience preferred.  Demonstrated ability to manage all aspects of terminal care, health team service coordination, and documentation.

Team Coordinator (Full time) –Supports the Clinical Services Manager in their role of leading and managing a team providing direct care to patients and families. This includes triaging incoming calls, acting as primary liaison for transport and equipment needs, and tracking Medicaid and Medicare re-certifications. High school degree required; college experience and LPN preferred. Recent experience in medical records system for home care/hospice preferred. Should be able to demonstrate knowledge of medical terminology and advanced computer skills, including proficiency in Word and Excel.

Positions in Culpeper and surrounding area:

Chaplain (PRN) – Provides spiritual support services to patients and families reflective of patient/family’s desire/belief system and serves as consultant to Interdisciplinary Team for spiritual support issues and information. Requires a Master’s degree in Divinity from accredited seminary or school theology or appropriate certification in hospital or pastoral ministerial counseling.  Five years’ experience in pastoral care or counseling required.  Hospice experience preferred.

Registered Nurse (PRN) – Graduate of an accredited nursing school and licensed as a Registered Nurse in the Commonwealth of Virginia. Minimum of two years of recent, varied work experience as a nurse, Hospice experience preferred.
