RN Case Manager - Successful candidate must be RN licensed in Virginia, 2+ years relevant experience required along with demonstrated ability to manage all aspects of terminal care, health team service coordination, documentation and case management.  Hospice experience preferred but not required.

Certified Nursing Assistants Needed -

        Field:  FT, serving Charlottesville and surrounding counties

        Hospice House:  FT (M-F / 3-11)

        Hospice House:  PRN (2 positions) every other weekend (7-3 and 11-7)

Must have current Virginia certification.
