A Pre-Hospice Program
to Enhance the Quality of Life
When you or someone you love is diagnosed with a terminal illness, your life is affected in many ways. A major concern is finding appropriate care and assistance in dealing with practical needs.
Transitions is a volunteer-based program that provides trained and experienced volunteers to assist individuals facing a terminal illness and their families.
Working with referral sources such as area cancer centers, home health agencies, discharge planners and physicians, a Transitions client coordinator works with the client and their family to identify and engage avenues of support according to need.
Transitions is available at no charge to any resident of Charlottesville and surrounding counties who has been diagnosed with a life-limiting illness and who may or may not be undergoing curative treatment. It is available wherever the client resides, whether in their own home or in an assisted living center or nursing facility.
Transitions offers case management, while our trained volunteers provide:
- Companionship
- Help with errands
- Transportation to physicians offices or treatment
- Respite break for family members and caregivers
